This page will contain the form for applying for aid. It will contain input boxes for type of aid requested, quantity of aid type requested, and date by which aid is needed. When submitted, the aid request will receive an class number and record the Aid Request table. Also, when this form is submitted, the system will return a page that confirms the submission and shows the ID number for the victim’s records.
